SPECIAL ANNOUNCEMENTS

2009 SCHOLARSHIP APPLICATION – KIDDO’S CLUBHOUSE FOUNDATION

 

Who may apply?

A family applying for this scholarship must:

·         Currently live in the state of Georgia

·         Demonstrate the need for funds due to lack of insurance, lack of insurance coverage, lack of other funding sources

·         Be applying for a child under the age of 21

·         Not have received a scholarship from Kiddos’ Clubhouse Foundation within the past two (2) years

 

What is the amount of the scholarship?

The scholarships to be awarded will provide $2,500 for payment of therapeutic services or equipment payable directly to the clinic where your child has received the services, licensed therapist or business or the equipment vending company that supplies the equipment. 

 

What is the process?

·         You may download the application and type your responses on your computer once the form is complete, please print it and sign it.  You may also print it out and fill it in by hand and sign it.

·         Applications will NOT be considered in the order in which they are received.  All applications will be considered at the same time.

·         All applications must be postmarked by September 30th, 2009

·         Completed applications may be mailed to:

·         Please allow 60 days past deadline date for processing, review and decision

·         All applicants will be notified in writing of the status of their application within 60 days past the deadline date

 

Guidelines

·         The monies may NOT be used to pay insurance premiums or medical bills. 

·         All invoices for therapy services provided or therapy equipment must be submitted to Kiddos’ Clubhouse Foundation.  Funds may not be used for previous outstanding balances and must be for services past scholarship award date.

·         Applications will only be accepted by mail.  Applications that are sent via email or fax will not be considered.

·         Applications that are incomplete or received without all required documentation will not be considered.

Champions for Children

 

Award:             Champions for Children

Amount:           $2,600 per year (maximum)

 

Eligibility:

  • Children 21 and under
  • Must live at home
  • Have a physical, cognitive, developmental or medical disability
  • Current Medicaid recipients not eligible
  • Those who do not meet the eligibility requirements for the TEFRA/Katie Beckett Medicaid Program

 

Services provided:

  • Respite Care
  • Specialized medical supplies and equipment
  • Medical services
  • Therapies (Occupational, Physical and Speech)
  • Dental and optical services
  • Travel reimbursements for medical appointments
  • Recreational/therapeutic activities
  • Other services related to the child’s disability not covered by other payment sources

Free or Discounted Telephone Service for Families in Need

 

Lifeline Benefits

Lifeline Assistance is part of a program that was created by the government to provide discounted or free telephone service to income-eligible consumers. To help bring this important benefit, SafeLink Wireless is proud to offer Lifeline Service. Through our Lifeline Service you will receive FREE cellular service, a FREE cell phone, and FREE Minutes every month! SafeLink Wireless Service does not cost anything – there are no contracts, no recurring fees and no monthly charges.

 

Any Minutes not used will roll-over. Features such as caller ID, call waiting and voicemail are all also included with the service. Additional minutes (TracFone Airtime Cards) can be purchased at any TracFone retailer Walmart, Walgreens, Family Dollar, etc). SafeLink Airtime Cards will be available soon.

Exact benefits, including the number of free Minutes received, depends on the state the applicant lives in.

 

How to Qualify

The process to qualify for Lifeline Service depends on the State you live in. In general, you may qualify if...

  

1.     You already participate in other State or Federal assistance program such as Federal Public Housing Assistance, Food Stamps and Medicaid.

OR

2.     Your total household income is at or below 135% of the poverty guidelines set by your State and/or the Federal Government.

AND

3.     No one in your household currently receives Lifeline Service through another phone carrier.

 

4.     You have a valid United States Postal Address. In order for us to ship you your free phone you must live at a residence that can receive mail from the US Post Office. Sorry, but P.O. Boxes cannot be accepted.

 

In addition to meeting the guidelines above you will also be required to provide proof of your participation in an assistance program, or proof of your income level.

Prescription Assistance from Free Medicine Foundation

 

Free Medicine Foundation brings together America’s pharmaceutical companies, doctors, other health care providers, hospitals, social services offices, government social service offices, patient advocacy organizations, and community groups to help qualifying patients get the medicines they cannot afford through public or private programs that’s right for them. It is our mission to increase awareness and boost enrollment of those who don’t know they qualify.  Who may be approved for Free Medicine Enrollment?

·         People of ALL AGES nationwide

·         Seniors and retired people

·         Students

·         Families

·         People on Social Security/Medicare with no outpatient prescription coverage

·         People on Social Security Disability or SSI, with no prescription coverage

·         People on a fixed income

·         Single people

·         Working people

·         People without jobs

·         People who are taking care of loved ones and paying for their prescription medication because they can't

 

 Requirements for Free Medicine Enrollment are:

 

1.       People who have no public or private prescription drug coverage

2.       People with household incomes at a level that paying for medicine at retail prices poses a hardship.

3.       People who make too much money to qualify for public assistance e.g. Medicaid

·         General Income Eligibility Criteria: Individuals with family incomes ranging from below the national poverty level up to $38,000, and in some cases, families with annual incomes as high as $80,000 can receive free drugs. ($39,200 for individuals, $52,800 for couples, or $80,000 for a family of four.) Each sponsored drug has it's own eligibility criteria.

 

Prescription Drugs: Hardship Circumstances may also allow Free Medicine Eligibility:

 

·         People who have maxed out their prescription insurance coverage

·         People on Medicaid, (state public assistance) but have prescriptions Medicaid does NOT cover or have exceeded the prescription drug limit for this calendar year

·         People on Medicare with no outpatient prescription coverage

·         Some, but not many sponsors offer help for people who cannot afford their co-insurance fees.

·         People who have reached a cap on their outpatient prescription drug insurance coverage

·         People who have medical insurance coverage, but NOT prescription coverage

·         On occasions, some sponsors will make an exception on a case-by-case basis or in extreme or extenuating circumstances. It never hurts to ask. One thing you should know is that there is something more important than income that sways drug companies in letting you qualify for their patient assistance... Hardship! Even if you do not qualify financially, you may still qualify for patient assistance programs if you demonstrate financial hardship. Some circumstance that might qualify you on the hardship issue include: job turnover, divorce, other catastrophic events. If you think you would like to apply, we will be diligent in our efforts to assist you. You really have nothing to lose. We have seen many people who would be considered "middle-class" status enrolled on the hardship basis.

Funding Opportunity for Special Needs Children

The M.O.R.G.A.N. Project is a non-profit organization, established by Robert & Kristen Malfara in honor of their precious son Morgan.  Their mission is to promote awareness of and support for parents caring for their special-needs children, and to enhance the quality of life for these special families.

 

QUALITY-OF-LIFE GRANT PROGRAM
The QOL grants will provide families with a wide variety of supports related to their medically fragile child with physical special needs. Only 1 grant application request per recipient per 12 month period will be considered. Please click here for application form in Word or PDF format.
