Instructions for completing the Public School to Public School Transfer form (2008)

To request a transfer to a public school within the Savannah-Chatham County Public School System, complete the transfer request form, providing all necessary information.

 

Copies of the form are available online at www.doe.k12.ga.us/sb10.aspx  and also in Room 204 of the Board of Education.

 

Sign and date the completed form and mail, fax, or deliver it to the Department for Exceptional Children at the address listed below by September 5, 2008.

Each and every parent request for a transfer will be carefully considered. Before a request can be approved, it must be determined if the school has adequate capacity and if the programs provided align with the student’s IEP. Parents will be notified as soon as possible of the decision. If the request is approved, the student is guaranteed a seat at the receiving school until the student completes the grades at that school or graduates. The parent is responsible for transportation to the new school.

 

To request a transfer to a public school in a different school system, complete the Georgia Public School to Public School Transfer Request form and take or mail the form to that school system’s central office. If that system accepts the student and approves the request, the parent is responsible for transportation to the new school.

 

Student Eligibility Criteria

 

·         The student’s parents currently reside within Georgia and have been Georgia residents for at least one calendar year; and

·         The student spent the prior school year (2007-2008) in attendance at a Georgia public school in grades K-12 and was in attendance on both the October 2007 and March 2008 FTE counts; and

·         The student was served under an Individualized Education Program (IEP) written by the school in accordance with federal and state laws and regulations at any point during the 2007-2008 school year.

 

The transfer form is available at http://public.doe.k12.ga.us/sb10.aspx.  Select “Transfer Request Form” in the “Public School Choice” box.  This form has two pages, however, parents only need to complete page one; page two is for department use.
