3 Steps to Register Your Child for School

 

1.  Complete the District Registration Packet
 
The District Registration Packet (PDF) consists of the following forms: 
Student Registration Form, Safe Schools Questionnaire (required for grades 6 -12), Opt-Out Notification Form, Code of Conduct/Attendance Truancy Form, and the Release of  Student Records Form.
 
All students who are new to the school district or re-enrolling need to complete the district registration packet.  Students continuing in the same school or rising to their zoned middle or high school do not need to complete the packet.  Students transferring to another school in the district due to an address change or admission in an academy/magnet program need only to submit a district registration packet and current proof of address to the new school.


2.  Obtain Required Documents

 
Savannah-Chatham County Schools must have all required documents upon registration for students to be properly enrolled and to be scheduled properly.

  • Birth Certificate (certified copy)
    A copy of the child's original birth certificate is required.  A copy of the birth certificate may be requested from the Chatham County Health Department at 912-356-2441 or the Georgia Department of Human Resources Vital Records at 404-679-4701.  Birth certificates for children born in other states may be obtained by contacting the Vital Records Department within the state.
  • Georgia Certificate of Immunization  (Form 3231)
    A current and completed Georgia Department of Human Resources Form 3231 is required showing immunizations for measles, mumps, rubella, polio, whooping cough, tetanus, diphtheria, hepatitis B, and varicella.  The form 3231 may be obtained from the Chatham County Health Department or a private physician.
  • Georgia Ear, Eye, and Dental Certificate (EED, Form 3300)
    A current and complete ear, eye and dental exam is required.  The form 3300 may be obtained from the Chatham County Health Department or a private physician.
  • Proof of Address
    One of the following current documents is necessary to verify proof of address:  mortgage statement; lease or rental agreement; governmental agency mail; gas, water or electric bill.  A document is current if it is dated within 30 calendar days of the date the registration application is presented to the school.   (Note: Parent(s)/Legal Guardian(s) who do not have a current proof of residence in their name must complete a Residency Affidavit Form).
  • Social Security Card
    A copy of the student's social security card is requested to verify the social security number.  For students interested in Hope eligibility, the social security number is required.
  • Recent Report Card / Transcript
    The student's most recent report card or transcript will ensure the student is scheduled properly.
     

3.  Take Documentation to Assigned School


Once the registration packet has been completed and the required documents obtained, take the documentation to the school to complete the registration process.

All students report to school for the first day of classes on Tuesday, September 2, 2008.
