Proof of Address (All Students)

One official document indicating residence within the attendance area must be provided to the school.The following is a list of acceptable of documents that may be requested for verification of residency:
- A current mortage statement
- A current utility bill (gas, water or electric) Please note that telephone and cable bills are not accepted
- A current lease or rental agreement (month to month)
- Current governmental agency mail (county, state, or federal)
All documentation must be current. (Last 30 days)
Students, parents, and/or legal guardians are responsible for reporting changes of address to school principals immediately upon moving or knowing that moves will take place.
Residency Affidavit Process
Effective beginning January 2006:
When the parent/legal guardian is not the resident occupant at a given address, the following items are required for proof of address documentation:
1. The Residency Affidavit Form (print form) must be signed and notarized by the parent/legal guardian and the residence owner. Notary services are available at each school, please contact the school for more information.
Please note that one affidavit per school is required.
AND
2. The residence owner must provide ONE of the following items in the name of the residence owner:
- A current mortage statement
- A current utility bill (gas, water or electric)
- A current lease or rental agreement (month to month)
- Current governmental agency mail (county, state, or federal)
Current is defined as within the previous 30 days. Cable or telephone bills are not acceptable.
