Students who want to tryout for an Athletic Team at Coastal MS must have a Valid/Current Sports Physical on file with the Athletic Department. All FORMS must be completed by Student, Parent(s), and Physician for the Packet to be COMPLETE. The Sports Physical Packet will not be accepted unless all FORMS and SIGNATURES are completed.
